Her website does not give a specific form, but Edrienne requires significant screening before she would see a new client. We emailed back and forth a few times and fortunately I was able to pass her screening requirements. She seems to be in high demand and it took several days to get email responses at times, but she did always eventually respond. We made arrangements to meet at my hotel and communication was very good. She arrived at my hotel room on time and was a truly beautiful sight!
